Attention families of school athletes entering grades 6-12: completed, physician-signed (on or after 4/15/2026) sports physicals for the upcoming school year can now be uploaded at montroseathletics.com. Click on Registrations, then Athletes, then Sports Forms, and follow the online instructions. You can complete and sign all of the form’s parent sections online, then upload your athlete’s valid, physician-signed sports physical. *Parent/athlete information does not need to be filled out on the form if you are completing the information online* Please reach out to Kim DiMeglio at kdimeglio@montroseschools.org if you have any questions.
